Doorway to Children’s Long-Term Support Services

Threshold provides a unified point for intake, application and eligibility determination for children’s long-term support (CLTS) services for families in Adams, Columbia, Dane, Green, Jackson, Jefferson, Kenosha, LaCrosse, Lafayette, Marquette, Monroe, Ozaukee, Racine, Rock, Walworth, Washington and Waukesha counties. Effective November 9, 2015, CWT no longer provides eligibility determination for children applying for Autism Treatment Services funded by the CLTS-MA Waiver because the Department of Health Services is transitioning the Autism Treatment Benefit to Medicaid. If a child diagnosed with Autism Spectrum Disorder wishes to apply for Katie Beckett Medicaid to obtain treatment or apply for CLTS-MA Waiver for other supports and services, the child’s family can continue to work with CWT for intake and eligibility determination.
